Abstract
Since the introduction of microscope in, surgery there have been much imp:- =vements in the technique, instrumentation and technical expertise, and no,, :ijays, the microsurgery becomes popularized in the field of reconstructive pla6iic surgery.
The transfei of free flap by microsurgical anastomos?s is now recognized as having many advantages over conventional pedicle flaps.
The advantages of free flaps are as follows:
1) This method saves time with a single operation and a short hospitalization.
2) It improves the comfort of the patient, and allows early ambulation.
3) It provides a well vascularized flap, because it is an "arterial flap" based on a well defined arteriovenous pedicle.
4) It diminishes the risks of donor site infection, because the donor site is primarily closed at the end of the operation.
5) It allows elevation of the limb for preveution of edema and joint stiffness.
6) The complete skin cover obtained allows bone, tendon and nerve surgery to be performed primarily.
We report a case of axillary scar contracture satisfactorily corrected by a microvascular free flap transfer frntn groin.
We made the anastomosis between right superficial circumflex iliac vessels of the flap and right lateral thoracic vessels of the recipient site by interrupt 10-U nylon sutures under the 16x operative microscope.
The postopecative course was unevenful without any evidence of vascular thrombosis or partial necrosis of the flap.
